摘要目的 探讨一种新型自体肋软骨移植物矫正唇裂继发鼻畸形的临床效果.方法 2013年9月至2016年3月,西安交通大学第一附属医院整形美容颌面外科收治＞16岁中到重度单侧唇裂继发鼻畸形患者38例,采用新型“伞状”自体肋软骨移植物矫正,对于合并鞍鼻和患侧鼻翼基底凹陷者,同时行自体肋软骨隆鼻和梨状孔边缘填充术.术后半年,对患者鼻外形及满意度进行评估.结果 38例患者术后均无感染、血肿、软骨外露等并发症.术后半年评估,患者鼻尖形态、位置、鼻背形态、鼻孔形状、鼻底外形均较治疗前有明显改善,其中以鼻尖形态、位置和鼻孔形状改善最为明显.患者对治疗后外形及功能均满意,家属对治疗后外形评价为基本满意.结论 新型“伞状”自体肋软骨移植物可有效改善唇裂继发鼻畸形,是一种可靠的唇裂继发鼻畸形矫正方法.

abstractsObjective To observe the effects of a novel type ‘umbrella’ graft in secondary cleft rhinoplasty.Methods 38 secondary cleft patients with moderate to severe nasal deformity that were older than 16 years in our department from September 2013 to March 2016 were received rhinoplasty with novel type ‘umbrella’ graft.Nasal augmentation and piriformaperture filling with autologous rib cartilage was applied in patients with saddle nose or nasal base depression.The appearance and patient's satisfaction were assessed at 6 months after surgery.Results There were no complications such as infection,hematoma and graft exposure in all 38 patients with rhinoplasty.The nasal tip,dorsum,nostril and nasal base were improved significantly 6 months after surgery.Especially the nasal tip and nostril,most of them,became almost normal.The patients' self-evaluation of the appearance and function were satisfying.Conclusions The novel type ‘umbrella’ graft can improve the deformity of cleft nose significantly,especially the appearance of the nose tip and nostril,which is a reliable method.
