532 nm皮秒与调Q纳秒Nd∶YAG激光治疗脂溢性角化病的效果
Clinical effects of 532 nm picosecond and nanosecond Nd∶YAG laser in treatment of early seborrheic keratosis
摘要目的:比较532 nm皮秒与纳秒Nd∶YAG激光治疗脂溢性角化病的疗效及不良反应。方法:2019年1月至2020年12月,上海市奉贤区皮肤病防治所就诊的脂溢性角化病患者30例,男12例、女18例;年龄30~65(32.2±4.3)岁;均选择直径1~2 cm皮损,同一皮损处一半用532 nm皮秒激光、另一半用532 nm调Q纳秒Nd∶YAG激光进行自身对照,波长532 nm,观察皮损面积与色素消退,判断治疗效果。治疗后3个月随访。结果:试验组和对照组治疗有效率分别为73.33%(22/30)、53.33%(16/30),两组比较差异无统计学意义( P>0.05);但试验组疼痛评分、痂皮脱落时间低于对照组,色素沉着发生也低于对照组,满意度评分高于对照组,差异有统计学意义(均 P<0.05)。 结论:532 nm皮秒激光和调Q纳秒Nd∶YAG激光治疗脂溢性角化病疗效差异无统计学意义,532 nm皮秒激光不良反应发生率和治疗满意度显著优于调Q纳秒Nd∶YAG激光。

abstractsObjective:To compare the efficacy and adverse reactions of picosecond and nanosecond Nd∶YAG laser 532 nm in the treatment of seborrheic keratosis.Methods:A total of 30 patients with seborrheic keratosis were enrolled in this study. The rash size was more than 1-2 cm. Half of them were treated with picosecond laser and the other half with nanosecond Q-switched Nd∶YAG laser by wavelength of 532 nm. The treatment effect and adverse reactions were evaluated by observing the area and the disappearance of pigment. The patients were followed up for 3 months after treatment.Results:The total effective rate was 73.33% in the experimental group and 53.33% in the control group, with no significant difference between the two groups ( P>0.05), but the pain score, satisfactory score, scab shedding time and the incidence of pigmentation in the experimental group were lower than those in the control group, and the differences were statistically significant ( P<0.05). Conclusions:The 532 nm picosecond laser has a higher efficiency in treating seborrheic keratosis than 532 nm Q-switched Nd∶YAG, but it has no statistical significance. However, the self satisfaction is higher than that of the control group; the pain score, scab shedding time and the incidence of pigmentation are lower than those of the control group, with statistical significance. Therefore, picosecond 532 nm laser treatment of early seborrheic keratosis is worthy of clinical promotion.
