摘要目的:探讨基于面部脂肪室解剖的美学分区在脂肪注射面部整形美容中的应用效果及意义。方法:将既往的面部10个美学单位49个美学亚单位修正为10个美学单位39个美学亚单位，对2018年1月至2020年1月在联勤保障部队第940医院接受面部脂肪注射的48例面部衰老及面部轮廓不佳女性患者(年龄19～68岁，平均43岁)，按照修正后的面部美学分区进行评估，然后实施深、浅层脂肪室的脂肪颗粒或(和)纳米脂肪移植。对各美学单位脂肪注射量进行统计，对并发症和临床效果进行随访。结果:48例患者，注射颗粒脂肪2～50 ml，纳米脂肪1～10 ml；各美学单位脂肪移植量：额部3～40 ml、颞部4～20 ml、眉部2～4 ml、眼眶部3～8 ml、鼻部3～7 ml、鼻唇沟部2～10 ml、颊部4～10 ml、唇部2～6 ml、颏部2～7 ml、耳垂1～3 ml。48例移植颗粒脂肪2～50 ml，平均20 ml,纳米脂肪1～10 ml，平均4 ml。术后7～14 d消肿，无一例出现感染、硬结、包块等并发症。术后随访7 d至2年，面部年轻化、轮廓美化效果均显著，皮肤质地、光泽度亦明显改善，患者均满意。结论:以面部美学分区为指导，实施深、浅脂肪室的精细化脂肪移植术，有助于有的放矢地实施脂肪注射面部年轻化及面部轮廓塑造，可提高临床治疗效果。

abstractsObiective:To explore the application and significance of facial aesthetic unit in the fat injection for facial aesthetic surgery based on the facial fat chamber anatomy.Methods:We revised the facial cosmetic units from ten cosmetic units and fourty-nine cosmetic subunits to ten cosmetic units and thirty-nine cosmetic subunits. From January 2018 to January 2020, In Joint Service Support Force No. 940 Hospital, 48 patients who accepted facial fat injection were evaluated by our facial cosmetic units. The autologous fat grafts was carried out both in deep and superficial fat chamber.Results:Of 48 patients, the volume of transplated granular adipose was from 2 ml to 50 ml, with an average of 20 ml, and the volume of transplated nano fat was from 1 ml to 10 ml, with an average of 4 ml. The post-operation swelling continued for 7-14 days. The complications, such as infection, hematoma and scleroma, did not happen. All patients were followed up from seven days to two years with satisfactory facial rejuvenation and contour improvement. The skin texture was also improved.Conclusions:With the reference of facial aesthetic units, the facial fat injection can be carried out both in the superfacial and deep fat chamber to improve facial rejuvenation and contour.
