摘要目的 探讨鼻腔鼻窦内翻性乳头状瘤的诊断及治疗.方法 对2000年9月至2005年4月行鼻内镜下鼻腔鼻窦内翻性乳头状瘤切除术患者的临床资料进行回顾性分析.26例患者术前均行鼻内镜检查,鼻腔鼻窦CT检查和组织病理学检查确诊为鼻腔鼻窦内翻性乳头状瘤,于局麻鼻内镜下鼻腔鼻窦内翻性乳头状瘤切除术:切割吸引器切除瘤体,开放病变鼻窦,清除窦内瘤体及分泌物,肿瘤基底部及可疑病变处辅以射频治疗,病变部分累及上颌窦者联合下鼻道开窗术,广泛累及上颌窦者联合柯-陆式手术.术后随访1-3年.结果 26例患者复发2例,治愈率92.2%.结论 根据症状、体征、组织病理学检查可确诊鼻腔鼻窦内翻性乳头状瘤,CT检查可确定肿瘤范围帮助选择术式;辅以射频处理术野的鼻内镜下鼻腔鼻窦内翻性乳头状瘤切除术有利于彻底切除肿瘤,广泛累及上颌窦者联合柯-陆式手术均可达到彻底切除肿瘤的目的 ,可作为鼻腔鼻窦内翻性乳头状瘤的首选术式.

abstractsObjective To discuss the method of diagnosis and therapy of sinonasal inverted papilloma.Methods Endoscopic sinonasal inverted papilloma excision of the clinical data that were from September 2000 to April 2005 were retrospectively analynsed.Twenty-six eases were diagnosed with sinonasal inverted papilloma by nasal endoscopic exmanation,nasal sinus CT examination and histopathology before operation,which all were cured with endoscopic sinonasal inverted papilloma excision:the main of tumors was cutted through attract cutting;the ill sinus were opened;the secretions and tumor in sinus were cleared;tumor-suspicious lesion and the bottom of tumor wad dealed with radio-frequency therapy;illness involving some of the maxillary sinus were dealed with inferior nasal meatus fenestration;tumors involving a wide range of the maxillary sinus were dealed with the caldweU-luc operation.All of the cases were followed up from 1 to 2 years.Results Two cases reeured in 26 eases;the cure rate of endoscopic surgery in sinonasal inverted papilloma was 92.2%.Conclusions According to the conclusions of the symptoms,signs,histopathological examination,sinonasal inverted papilloma can be final diagnosed;CT screening may help determine the scope of the tumor surgical option;radio frequency dealing with the endoscopic surgical field sinonasal inverted papilloma Surgery can be profit to remove the tumor completely;Endoscopic surgery in inverted papilloma of the nose and paranasal sinuses united with the caldwell-luc operation can achieve the complete removal of the tumor.It can be the preferred method for endoscopic surgery to cure inverted papilloma of the nose and paranasal sinuses
