摘要目的：评估远程心电监测在心律失常、心肌缺血患者临床诊断中的应用价值。方法选取2012年5月至2014年5月于河南大学淮河医院门诊接受远程心电图监测的64例确诊为心脏不适或心脏病的患者，同时对所有患者实施12导联心电图检查，选取其中40例患者做24 h动态心电图检查，对比观察常规导联心电图检查、24 h动态心电图及远程心电监测在诊断心律失常、心肌缺血方面的检出率。结果64例患者远程心电监测提示心律失常52例，占81.3%，其心律失常检出率明显高于常规心电图的59.4%，两种不同诊断方式比较差异有统计学意义(P<0.05)，且远程心电监测ST-T段改变检出率为17.2%，同样优于常规心电图的7.8%(P<0.05)；远程心电检出心律失常14例，检出率为33.3%，稍高于动态心电图的28.6%，但差异未见统计学意义(P>0.05)。结论为提高心律失常及心肌缺血的检出率，可将远程动态监测作为常规心电图检查与24 h动态心电图检查的辅助诊断手段。

abstractsObjective To evaluate the application value of remote ECG monitoring in clinical diagnosis of arrhyth-mia, myocardial ischemia. Methods Sixty-four patients diagnosed with heart disease or heart discomfort accepted remote ECG monitoring in huaihe hospital of Henan university from May 2012 to May 2014 were selected, they were all given the 12 lead ECG examination, and 40 patients given the 24 h dynamic electrocardiogram, the positive rate of myocardial ische-mia and arrhythmia comparative diagnosed by routine lead electrocardiogram inspection, detection rate 24 h dynamic elec-trocardiogram and remote ECG monitoring were compared. Results Among the 64 cases, 52 cases of arrhythmia detected by the remote ECG, accounting for 81. 3%, arrhythmia detection rate was significantly higher than that of conventional ECG (59. 4%), the difference was significant (P<0. 05), and the remote ECG monitoring of ST-T segment change detection rate (17. 2%) was better than that of conventional ECG (7. 8%),P<0. 05;Remote ECG arrhythmia was detected in 14 cases, the positive rate was 33. 3%, slightly higher than dynamic electrocardiogram (28. 6%), the differences were not significant (P>0. 05). Conclusions In order to improve detectable rate of arrhythmia and myocardial ischemia, remote dynamic moni-toring can be as auxiliary diagnostic means routine ECG examination and 24 h dynamic electrocardiogram examination.
