摘要目的：探讨应变率成像技术在评价高血压患者局部心肌舒缩功能中的临床价值。方法140例受检者入选本研究，其中高血压非左室肥厚（NLVH）50例，左室肥厚（LVH）50例，对照组40例。所有受检者均行常规超声心动图检查，同时记录二尖瓣瓣环侧壁的运动速度，取心尖四腔、心尖两腔、心尖左室长轴切面的心肌应变率曲线，测定各室壁节段的收缩期、舒张早期和心房收缩期的最大应变率（ SRs、SRe、SRa）。结果与对照组比较，LVH组除前、后间隔基底和中间段以及后壁基底段 SRs有显著性降低外（ P＜0.05），余各室壁节段的SRs差异未见统计学意义；除左室侧壁中间段外，高血压组各室壁节段的SRe均较对照组显著降低（ P＜0.05）；高血压组除在下壁、后壁基底和中间段以及后间隔中间段SRa有显著性增加外（ P＜0.05），余各个室壁节段的SRa差异未见统计学意义。结论应变率成像技术（ SRI ）是一项准确定量评价高血压患者左室整体和局部收缩、舒张功能的无创性新技术。

abstractsObjective To investigate the clinical value of strain rate imaging ( SRI) in detection of regional func-tion left ventricular in essential hypertension (EH) patients.Methods One hundred and forty subjects were included in our study, 50 hypertension patients without LVH , 50 hypertension patients with LVH and 40 normal subjects.All subjects were examined by routine echocardio-gram,and were recorded with peak velocity at mitral annular during systole , diastole and atrium contraction(Vs,Ve,Va).Longitudinal strain rate(SR) during systole, diastole and atrium contraction(SRs, SRe, SRa) were measured along LV long axis views on cardiac apex four chamber , two chamber and apex .Results Com-pared with normal subjects , LVH patients had no significant difference in SR on all of the walls in systole , except the ven-tricular septum and the basal of the posterior wall .Except for the middle of the lateral wall , the SR on all of the walls in early diastole decreased obviously in hypertension patients (P<0.05).Hypertension patients had no significant difference in SR on all of the walls in atrium contraction , except the inferior , posterior wall and the basal of posterior septum .Con-clusions SRI can assess the left ventricular global and regional functions in patients with hypertension .Therefore , SRI is a valuable quantitative noninvasive technique .
