微型快速成型导向模板辅助经后路寰枢关节突螺钉置钉偏差分析
Deviation factors analysis of posterior atlantoaxial transarticular screw placement assisted by mi-cro rapid prototyping guided template
摘要目的:验证微型快速成型导板辅助经后路寰枢关节突螺钉置钉的准确性及偏差分析。方法随机选择12例受试者寰、枢椎 CT 连续扫描数据,利用 Mimics 10.01重建后导入 UG 7.0设计偏差检测板及枢椎后表面互补基板。用桌面3D 打印机器制作颈椎及导板模型,利用导板、直径3 mm 克氏针在3D 颈椎模型模拟置钉。最后利用游标卡尺在偏差检测导板上测量出钉点距 X 轴、Y 轴位移,克氏针置入长度,并与计算机测量数值进行比较。结果12例患者24枚螺钉均未穿过骨皮质,通过配对样本 t 检验,游标卡尺测量组与计算机测量组比较差异未见统计学意义(P >0.05)。结论使用微型导板在减少设计和制造时间的同时能有效引导后路寰枢关节突螺钉置入,提高了该手术的安全性,同时为数字化导航手术的实现奠定了基础。

Methods This study was based upon CT scans of 12 subjects atlantoaxial,using the mimics 10. 1 to reconstruct and then import UG 7. 0 design deviation detection plate and the axial inverse plate. Cervical model and guide plate were made by desktop 3D printing machine,guide plate and the Kirschner wire of 3 mm diameters were used to simu-late the cervical screw drilling,and the vernier caliper were applied to measure the Kirschner wire length in the vertebra,displacement in the X axis and Y axis in deviation detection plate. The data with the comput-er measurement was compared. Results Twenty-four screws of 12 patients did not through cortical bone. By paired samples t-test,there was no significant difference(P >0. 05)in deviation of the vernier caliper measuring group and the computer measurement group. Conclusions Mini guiding template can reduce the designing and manufacturing time and improve the surgery safety of posterior atlantoaxial screw place-ment. At the same time,it lays a foundation for the realization of digital navigation surgery.
