肱骨短缩结合锁定钢板技术对肱骨干骨折多次术后骨不连的治疗效果
Gffect of shortening of humerus combined with locking plate technology on bone ununion after multiple operations for humeral shaft fracture
摘要目的:探讨肱骨短缩结合锁定钢板技术治疗肱骨干骨折多次术后骨不连的效果。方法选取2013年1月至2014年2月肱骨干骨折多次术后骨不连患者50例,均给予肱骨短缩结合锁定钢板技术治疗,术后不采取外固定,早期锻炼肩肘关节功能。比较手术前后患侧肩肘功能。结果50例患者均顺利完成手术,术后未见切口感染。随访6~24个月,切口均为一期愈合,肢体短缩2~6 cm。骨折均愈合,愈合时间6~15个月,无一例内固定失败。术后8个月,患侧 Hss 评分(93.9±5.0)、Con-stant-Murley 评分(92.7±5.7)高于术前,差异有统计学意义(P <0.05)。结论肱骨短缩结合锁定钢板技术可提高患侧肩肘关节功能,缩短骨折愈合时间,进一步改善预后。

Methods From January 2013 to February 2014,50 patients with bone ununion after multiple operations for humeral shaft fracture were selected,and were given shortening of the humerus combined with locking plate technology without external fixation after operation,and the training for the shoulder joint and elbow joint function was treated. The grade of the shoulder joint and elbow joint function of pretherapy and post-treatment of them was compared. Results All of the operations were completed successfully without infection of inci-sional wound of postoperation. Follow-up was 6-24 months,all primary healing of incision,the limb shortened 2-6 cm. All the fractures healed,and the healing time was 6-15 months without ant failures of internal fixation. After 8 months of postoperation,the grade of Hss(93. 9 ± 5. 0)and Con-stant-Murley (92. 7 ± 5. 7)were higher than those of preoperative,the differences were significant( P < 0. 05). Conclusions Shortening of the humerus locking plate technology can improve the side shoulder elbow function,and shorten fracture healing time,and may further improve the prognosis.
