摘要目的 探讨单孔腹腔镜疝囊高位结扎术和小切口手术治疗小儿腹股沟斜疝的临床效果.方法 回顾性分析在医院接受治疗的220例小儿腹股沟斜疝患者的临床资料,按照不同手术方式分为两组,采用单孔腹腔镜疝囊高位结扎术治疗的110例患者作为研究组,小切口手术治疗的110例患者作为对照组.将两组患者手术时间、切口大小、术中出血量、住院时间、住院费用、切口满意度、术后VAS评分、并发症发生情况及复发情况进行比较.结果 研究组患者双侧疝手术时间、切口大小显著短于对照组(P<0.05),术中出血量显著少于对照组(P<0.05),家属对切口满意度及住院费用显著高于对照组(P<0.05).研究组患者术后VAS评分1～3分比例显著高于对照组,>7分比例显著低于对照组(P<0.05).研究组患者并发症发生率(4.54％)显著低于对照组(18.18％)(P<0.05),研究组复发率(0.09％)与对照组(0.18％)比较差异未见统计学意义(P>0.05).结论 单孔腹腔镜疝囊高位结扎术治疗小儿腹股沟斜疝损伤小,术后并发症少,疼痛感低,且术中可以及时发现并处理对侧隐匿疝,但住院费用高,值得在临床上推广使用.

abstractsObjective To investigate the clinical effects of single hole laparoscopic hernia sac high ligation and small incision surgery on indirect inguinal hernia in children. Methods The clinical data of 220 patients with pediatric inguinal hernia were retrospectively analyzed,the patients were divided into two groups according to the surgery methods,110 patients treated with single lumen laparoscopic her-nia sac were in the study group,and 110 patients with small incision operation were in the control group. The operation time,incision size,intraoperative bleeding volume,hospitalization time,hospitalization expenses,incision satisfaction,postoperative VAS score,complications and recurrence were compared between the two groups. Results The time and length of incision was significantly shorter in the study group than those in the control group (P < 0. 05). The intraoperative blood loss was significantly less than that of the control group (P < 0. 05). The satisfaction rate and hospitalization cost of the family were significantly higher than those of the control group (P < 0. 05). The proportion of VAS score and 1 ～ 3 score in the study group was significantly higher than that in the control group,and the proportion of scores more than 7 was significantly lower than that of the control group (P < 0. 05). The incidence of complications in the study group (4. 54％)was significantly lower than that in the control group (18. 18％,P < 0. 05),and the recurrence rate (0. 09％)in the study group was not significantly differ-ent from the control group (0. 18％),P > 0. 05. Conclusions Single hole laparoscopic hernia sac liga-tion in the treatment of pediatric inguinal hernia patients has less injury,less postoperative complications, low pain,and the operation can timely detect and treat the bilateral hernia,but has high costs of hospital-ization,it is worth popularizing in clinics.
