摘要目的 探讨口腔癌切除术后颏下岛状瓣和吻合血管的游离皮瓣的不同修复方法的临床效果,同时评估术后1年患者生活质量并分析其影响因素.方法 对医院32例口腔癌患者术后临床效果进行分析,并以华盛顿大学生存质量量表中文第四版(UW-QOL)进行评估分别应用颏下岛状皮瓣及带血管吻合的游离皮瓣进行颌面部缺损重建的口腔癌术后患者,经过测评得分后对数据进行分析其影响因素.结果 32例口腔鳞状细胞癌术后患者中,采用颏下岛状瓣患者与采用吻合血管的游离皮瓣患者治疗效果比较,差异未见统计学意义(P>0.05),采用颏下岛状瓣和吻合血管的游离皮瓣患者的总体生活质量比较差异未见统计学意义(P>0.05),但单项评分之间存在差异.结论 两组患者术后临床效果及总体生活质量差异不大,临床中应结合患者情况,选择合适皮瓣修复方案.

abstractsObjective To investigate the clinical effects of two different repair methods on submental island flap and free flap of vascular anastomosis after resection of oral cancer , and to evaluate the quality of life and analyze the influencing factors after 1 year after operation .Methods The clinical effects of 32 patients with oral cancer were analyzed and the Chinese fourth edition of the Washington University Students'memory quality scale ( UWQOL) was used to evaluate the use of submental island flap and free flap with vascular anastomosis for reconstruction of maxillofacial defects .After the evaluation score, the data were analyzed for analyzing its influencing factors .Results Among the 32 patients with oral squamous cell carcinoma after surgery , there was no significant difference in the therapeutic effect between patients with submental island flap and patients with free flap anastomosed with blood vessels ( P>0.05 ) .There was no significant difference in the overall quality of life of the patients with submental island flap and free flap with vascular anastomosis ( P>0.05 ) , but there was difference in the single score between the patients .Conclusions There is no significant difference in the clinical effect and overall quality of two group of patients .Proper flap repair scheme should be choosed according to the patient's own situation .
