不同体位下磁共振扫描在踝关节周围韧带中的显示效果
Display effect of magnetic resonance scanning in different positions on the ligaments around the ankle
摘要目的 探讨不同体位下磁共振扫描(MRI)在踝关节周围韧带中的显示效果.方法 选取2016年12月至2017年12月大同市第三人民医招募的75例健康志愿者作为研究对象,分别行自然体位、完全背屈位、完全跖屈位MRI扫描.统计踝关节周围韧带影像学特征,比较外侧副韧带(距腓前韧带、距腓后韧带),内侧副韧带(胫舟韧带、胫跟韧带)及胫腓下联合韧带在不同体位MRI显示效果.结果 ①距腓前韧带在完全背屈-横断面、自然体位-横断面显示最佳,评3分者相对较多,分别占评分3分者38.96%(60/154)、41.55% (64/154),两者显示效果比较,差异未见统计学意义(P>0.05);②距腓后韧带在自然体位-冠状面、完全背屈-横断面显示最佳,评3分者相对较多,分别占评分3分者26.66%(52/195)、75.78%(72/195),两者显示效果比较,差异有统计学意义(P<0.05);③胫舟韧带在完全背屈-横断面、完全跖屈-冠状面显示最佳,评3分者相对较多,分别占评3分者29.62%(40/135)、39.26%(53/135),两者显示效果比较,差异未见统计学意义(P>0.05);④胫跟韧带在完全背屈-横断面及冠状面显示最佳,评3分者居多,分别占评3分者25.00%(47/188)、34.57%(65/188),两者显示效果比较,差异有统计学意义(P<0.05);⑤胫腓下联合韧带在自然体位-横断面、完全背屈-横断面显示最佳,评3分者居多,分别占评3分者28.67%(41/143)、10.55%(58/143),二者显示效果比较,差异有统计学意义(P<0.05).结论 不同体位MRI扫描对踝关节周围韧带显示效果不同,其中外侧副韧带以完全背屈-横断面显示效果最佳,内侧副韧带以完全跖屈-冠状面、完全背屈-冠状面显示效果最佳,下胫腓联合韧带以完全背屈-横断面显示效果最佳,可为临床制定合理有效干预措施提供依据.

abstractsObjective To investigate the display effect of magnetic resonance scanning (MRI) in different positions on the ligaments around the ankle.Methods Seventy-five healthy volunteers recruited in the Third People's Hospital of Datong from December 2016 to December 2017 were selected as the subjects,and the MRI scans of natural body position,complete dorsal flexion and complete metatarsal flexion were performed respectively.The imaging features of the ligaments around the ankle was recorded.The MRI display effect of different body positionsin lateral collateral ligament (anterior calf ligament,posterior calf ligament),medial collateral ligament (tibial ligament,tibial ligament) and tibiofibular ligament were compared.Results ①The display effect of ligamenta talofibulare anterius on complete dorsiflexion cross section and natural position-cross sectionwas the best.And the 3 scores were relatively more,accounting for 38.96% (60/154) and 41.55% (64/154) respectively.And the two showed no significant difference (P >0.05).②The display effect of ligamenta talofibulare anterius onnatural body position-coronal surface and complete dorsal flexion cross section was the best,and the 3 scores were relatively more,accounting for 26.66% (52/195) and 75.78% (72/195) respectively,and the difference was significant (P < 0.05).③ The display effect of ligamenta tibionaviculare on complete dorsal flexion cross section and complete metatarsal flexion-coronal surface was the best,and the 3 scores were relatively more,accounting for 29.62% (40/135),39.26% (53/135) respectively.There was no significant difference (P > 0.05).④The display effect oftibial heel ligament on complete dorsal flexion cross section and coronal plane was the best,and the 3 scores were relatively more,accounting for 25% (47/188) and 34.57% (65/188) respectively.The difference was significant in the two kinds (P < 0.05).⑤The display effect ofcombined tibial and peroneal ligament on natural body position-cross section and complete back bend cross section was the best,and the 3 scores were relatively more,accounting for 28.67% (41/143) and 10.55% (58/143) respectively,the difference was significant (P < 0.05).Conclusions The display effect of MRI scanning in different positions on the ligaments around the ankle is different.The display effect of lateral collateral ligament on complete dorsiflexion cross section is the best.The display effect of medial collateral ligament on complete plantar flexion coronal,complete dorsal flexion-coronal surface is the best.The display effect of joint ligaments of lower tibiofibula on complete dorsiflexion cross section is the best.It could provide the basis for the clinical formulation of rational and effective measures.
