MMP-14和c-myc在原发性肝细胞癌组织中的表达及其临床意义

Expression and clinical significance of matrix metalloproteinase-14 and c-myc in primary hepatocellular carcinoma

摘要目的 探讨原发性肝细胞癌(HCC)组织中基质金属蛋白酶-14(MMP-14)和c-myc的表达及其临床意义.方法 随机抽取2015年6月至2016年12月山西医科大学第一医院收治的HCC患者80例,均经组织病理确诊.另选取因外伤活检者20例(正常肝组织).应用免疫组织化学染色(SP)法检测80例HCC组织、80例HCC旁组织、20例正常肝组织中MMP-14和c-myc的表达情况.分析癌组织中MMP-14和c-myc表达与HCC转移、分期关系.采用Spearman相关分析法分析MMP-14与c-myc表达的相关性.结果 HCC组织中c-myc和MMP-14表达阳性率分别为88.75%、83.75%,高于正常肝组织的5.00%、5.00%及癌旁组织的23.75%、28.75%(P=0.000、0.024、0.000、0.012).无淋巴结转移者在HCC组织中MMP-14及c-myc表达阳性率低于有淋巴结转移者(P<0.05).病理分期为I~Ⅱ期的HCC组织中MMP-14及c-myc表达阳性率低于Ⅲ~Ⅳ期(P=0.009、0.018);HCC组织中的MMP-14及c-myc表达阳性率呈正相关(r=0.713,P=0.024).结论 HCC的发生发展与MMP-14及c-myc异常表达相关,且MMP-14和c-myc参与肿瘤的形成、发展、侵袭及转移过程,可作为判断HCC生物学行为的临床参考指标.

abstractsObjective To investigate the expression of matrix metalloproteinase-14 ( MMP-14 ) and c-myc gene in primary hepatocellular carcinoma ( HCC ) and their clinical significance. Methods A total of 80 patients with HCC admitted to the First Hospital of Shanxi Medical University from June 2015 to December 2016 were randomly selected and confirmed by histopathology. Another 20 patients (normal liver tissue) were selected for traumatic biopsy. Immunohistochemical staining (SP) was used to detect the expression of MMP-14 and c-myc in 80 cases of HCC, 80 cases of HCC and 20 cases of normal liver. The relationship between MMP-14 and c-myc expression and HCC metastasis and staging was analyzed in cancer tissues. The correlation between MMP-14 and c-myc expression was analyzed by Spearman correlation analysis. Results The positive rates of c-myc and MMP-14 expression in HCC tissues were 88. 75% and 83. 75%, respectively, which were higher than 5. 00%, 5. 00% of normal liver tissues and 23. 75% and 28. 75% of adjacent tissues ( P=0. 000 , 0. 024 , 0. 000 , 0. 012 ) . The positive rate of MMP-14 and c-myc expression in HCC tissues was lower than that in patients with lymph node metastasis ( P<0. 05 ) . The positive rates of MMP-14 and c-myc expression in HCC tissues with pathological stage I-II were lower than those in stage III-IV ( P=0. 009 , 0. 018 ) . There was a positive correlation between the expressions of MMP-14 and HIF-1 a ( r=0. 713 , P = 0. 024 ) . Conclusions The occurrence and development of HCC are related to the abnormal expression of MMP-14 and c-myc. MMP-14 and c-myc are involved in the formation, development, invasion and metastasis of tumors, and can be used as clinical reference indicators to judge the biological behavior of HCC.
