摘要目的:比较耻骨上经膀胱前列腺摘除术(SPP)与经尿道前列腺电切术(TURP)治疗大体积前列腺增生的的安全性和有效性。方法:采用随机对照方式，将长治医学院附属和济医院2012年1月至2018年1月收治的160例体积≥80 ml的前列腺增生患者作为观察对象，根据治疗方法的不同，将患者分为TUPP组和SPP组，每组80例，比较两组患者的手术时间、术中出血量、留置尿管时间、膀胱冲洗时间、住院时间以及术后6个月的最大尿流率(Qmax)、生活质量(QOL)和国际前列腺症状量表(IPSS)评分。结果:SPP组患者手术时间、术中出血量、留置尿管时间、术后膀胱冲洗时间和住院时间长于TURP组患者( P<0.05)；随访6个月后，TURP组患者的Qmax大于SPP组( P<0.05)，IPSS评分和QOL评分低于SPP组( P<0.05)。 结论:TURP治疗大体积良性前列腺增生患者具有手术时间短、出血量少、住院时间短的优点，同时能够提高患者的治疗效果，值得临床推广。

abstractsObjective:To compare the safety and efficacy of suprapubic transvesical prostatectomy (SPP) and transurethral resection of the prostate (TURP) in the treatment of massive prostatic hyperplasia.Methods:In this study, 160 patients with prostatic hyperplasia ≥80 ml treated in Heji Hospital Affiliated to Changzhi Medical College from January 2012 to January 2018 were selected as random observations. The patients were divided into TUPP group and SPP according to different treatment methods, and 80 patients in each group. The operation time, intraoperative blood loss, indwelling urinary catheter time, bladder irrigation time, hospital stay, and the maximum urine flow rate (Qmax), quality of life (QOL), and International prostate symptom scale (IPSS) score were compared between the two groups.Results:The operation time, intraoperative blood loss, indwelling catheter time, postoperative bladder irrigation time and hospital stay were longer in SPP group than those in TURP group ( P<0.05). After 6 months of follow-up, the Qmax of TURP group was greater than that of SPP group ( P<0.05), IPSS score and QOL score of TURP were lower than those of SPP group ( P<0.05). Conclusions:TURP has advantages of large surgical period, small amount of bleeding, and short hospital stay for patients with benign prostatic hyperplasia. At the same time, it can improve the treatment effect of patients, which is worth of clinical promotion.
