摘要目的:探讨经阴道彩色多普勒超声评价去氧孕烯炔雌醇片、炔雌醇环丙孕酮片治疗多囊卵巢综合征的效果。方法:抽取2017年6月至2019年6月于平顶山市中医医院接受治疗的多囊卵巢综合征患者80例，按随机数字表法分为A、B两组。A组38例采用去氧孕烯炔雌醇片治疗，B组42例采用炔雌醇环丙孕酮片治疗。两组均行彩色多普勒超声检查，比较两组治疗前后卵巢物理指标、卵巢动脉血流情况、性激素指标、临床症状及排卵、妊娠情况。结果:治疗前，两组卵巢内卵泡数(FN)、卵巢体积(OV)、卵巢间质面积(SA)比较差异未见统计学意义( P>0.05)；治疗后，两组FN、OV和SA值均小于治疗前( P均<0.05)，但两组间比较差异未见统计学意义( P>0.05)。治疗前，两组卵巢动脉舒张末期血流速度(EDV)、收缩期峰值血流(PSV)、阻力指数(RI)以及搏动指数(PI)比较差异未见统计学意义( P>0.05)；治疗后，两组EDV、PSV均低于治疗前，RI和PI均高于治疗前( P均<0.05)，但两组间上述指标比较差异未见统计学意义( P>0.05)。治疗前，两组血清总睾酮(TT)、黄体生成素(LH)/卵泡刺激素(FSH)比较差异未见统计学意义( P>0.05)；治疗后，两组性激素指标均低于治疗前( P均<0.05)，但组间比较差异未见统计学意义( P>0.05)。治疗后，两组临床症状及排卵率、妊娠率均发生明显改善( P<0.05)，但组间比较差异未见统计学意义( P>0.05)。 结论:利用彩色多普勒超声可以较直观的观察多囊卵巢综合征患者症状改善情况，去氧孕烯炔雌醇片和炔雌醇环丙孕酮片对治疗多囊卵巢综合征均有较好疗效。

abstractsObjective:To investigate the effect of desogestrel and ethinylestradiol tablets, ethinylestradiol and cyproterone acetate tablets in the treatment of polycystic ovary syndrome (PCOS) by transvaginal color Doppler ultrasound.Methods:Eighty patients with PCOS treated in Pingdingshan Hospital of Traditional Chinese Medicine from June 2017 and June 2019 were selected and divided into group A and group B by random number table method. Thirty-eight patients in group A were treated with desogestrel and ethinylestradiol tablets, while 42 patients in group B were treated with ethinylestradiol and cyproterone acetate tablets. All subjects were examined by color Doppler ultrasound. The ovarian physical indexes, ovarian artery blood flow, sex hormone indexes before and after treatment, and clinical symptoms, ovulation and pregnancy were compared between the two groups.Results:Before treatment, there was no significant difference in follicle number (FN), ovarian volume (OV), or ovarian stromal area (SA) between the two groups ( P>0.05). After treatment, the values of FN, OV and SA in the two groups were lower than those before treatment (all P<0.05), but there was no significant difference in these indexes between the two groups ( P>0.05). Before treatment, there was no significant difference in ovarian artery end-diastolic velocity (EDV), peak systolic velocity (PSV), resistance index (RI) or pulsatility index (PI) between the two groups ( P>0.05). After treatment, EDV and PSV of the two groups were lower than those before treatment, RI and PI were higher than those before treatment (all P<0.05); but there was no significant difference in these indexes between the two groups ( P>0.05). Before treatment, there was no significant difference in serum total testosterone (TT), luteinizing hormone (LH) / follicle stimulating hormone (FSH) between the two groups ( P>0.05). After treatment, the sex hormone indexes of the two groups were lower than before treatment (all P<0.05), but there was no significant difference between the two groups ( P>0.05). After treatment, the clinical symptoms, ovulation rate and pregnancy rate of the two groups were significantly improved ( P<0.05), but there was no significant difference between the two groups ( P>0.05). Conclusions:Color Doppler ultrasound can more intuitively reflect symptom relief in patients with PCOS. Desogestrel and ethinylestradiol tablets, and ethinylestradiol and cyproterone acetate tablets are effective in the treatment of PCOS.
