Effects of reverse-suspension traction combined with progressive resistance training on the recovery of Cobb angle of patients with adolescent idiopathic scoliosis

摘要目的:探讨反悬吊牵引联合渐进性抗阻训练对青少年特发性脊柱侧弯(AIS)Cobb角恢复的影响。方法:抽取2020年1月至2020年12月三门峡市中心医院收治的80例AIS患者作为研究对象,采用随机数字表法分为对照组和观察组,每组40例。对照组采用反悬吊牵引治疗,观察组采用反悬吊牵引联合渐进性抗阻训练治疗,均训练12周。对比两组训练后的总有效率、椎弓根对称度,训练前后的躯干位移距离、Cobb角、顶椎椎体旋转度、视觉模拟评分法(VAS)评分和脊柱侧屈肌力。结果:训练后,观察组治疗总有效率(90.00%,36/49)高于对照组(70.00%,28/40), P<0.05;观察组躯干位移距离、Cobb角、顶椎椎体旋转度和VAS评分均低于对照组( P均<0.05);左侧屈肌力和右侧屈肌力均高于对照组( P均<0.05)。训练后,观察组椎弓根对称情况优于对照组( P<0.05)。 结论:采用反悬吊牵引联合渐进性抗阻训练治疗青少年AIS,可明显改善患者的Cobb角、躯干位移距离顶椎椎体旋转度和疼痛感,提升其脊柱侧屈肌力,临床疗效显著。

abstractsObjective:To investigate the influence of reverse-suspension traction combined with progressive resistance training on the recovery of Cobb angle of patients with adolescent idiopathic scoliosis (AIS).Methods:Eighty patients with AIS admitted to Sanmenxia Central Hospital from January 2020 to December 2020 were enrolled as the research objects. According to random number table method, they were divided into control group and observation group, with 40 cases in each group. The control group was treated by reverse-suspension traction, while the observation group was treated by progressive resistance training based on the treatment of the control group. All of them were trained for 12 weeks. The total response rate after training, pedicle symmetry, trunk displacement distance before and after training, Cobb angle, apical vertebral rotation, visual analogue scale (VAS) score, and spine lateral flexor strength were compared between the two groups.Results:After training, total response rate of treatment in the observation group (90.00%, 36/49) was higher than that in the control group (70.00%, 28/40), P<0.05. After training, trunk displacement distance, Cobb angle, apical vertebral rotation and VAS scores in the observation group were lower than those in the control group (all P<0.05). After training, the left and right flexor strength in the observation group were higher than those in the control group (all P<0.05). After training, the pedicle symmetry in observation group was better than that in the control group ( P<0.05). Conclusions:The application of reverse-suspension traction combined with progressive resistance training in the treatment of AIS can significantly improve Cobb angle, trunk displacement distance, apical vertebral rotation, pain and spine lateral flexor strength, with significant clinical curative effects.
