非翻瓣上颌窦内提升微创种植术联合Bicon短种植系统治疗上颌后牙区单牙缺失的效果分析
Effect of flapless osteotome maxillary sinus elevation minimally invasive implant combined with Bicon short implant system in the treatment of maxillary posterior single tooth loss
摘要目的:探讨非翻瓣上颌窦内提升微创种植术联合Bicon短种植系统治疗上颌后牙区单牙缺失的临床效果。方法:抽取驻马店市中医院2017年1月至2021年1月收治的上颌后牙区单牙缺失患者82例,按照随机数字表法分为研究组与对照组,每组41例。研究组采用经牙槽嵴顶入路冲顶法行上颌窦内提升术联合同期Bicon短种植系统治疗,对照组采用翻瓣上颌窦内骨粉填塞联合延期种植普通种植体治疗。比较两组治疗效果、症状改善状况、种植体状况、视觉模拟评分法(VAS)评分、红色美学指数(PES)评分及并发症发生率。结果:研究组总有效率(97.56%,40/41)高于对照组(90.49%,33/41), P<0.05;两组边缘骨吸收量、上颌窦底提升高度比较差异未见统计学意义( P>0.05);研究组种植体完好率(92.68%,38/41)高于对照组(90.73%,29/41), P<0.05;术后3 d,研究组VAS评分低于对照组,PES评分高于对照组( P<0.05);研究组并发症发生率(7.32%,3/41)低于对照组(24.39%,10/41), P<0.05。 结论:采用非翻瓣上颌窦内提升微创种植术联合同期Bicon短种植系统治疗上颌后牙区单牙缺失,能缩短治疗时间,提高治疗效果,改善临床症状,还可提高种植体完好度,缓解术后疼痛,增强美观度,具备安全性。

abstractsObjective:To investigate the effect of flapless osteotome maxillary sinus elevation minimally invasive implant combined with Bicon short implant system in the treatment of maxillary posterior single tooth loss.Methods:A total of 82 patients with maxillary posterior single tooth loss treated in Zhumadian Hospital of Traditional Chinese Medicine from January 2017 to January 2021 were selected and divided into study group and control group according to the random number table method, with 41 cases in each group. The study group was treated by osteotome maxillary sinus elevation through alveolar crest approach combined with Bicon short implant system, and the control group was treated by flapping bone powder packing in maxillary sinus combined with delayed implantation of ordinary implants. The treatment effect, symptom improvement, implant status, visual analogue scale (VAS) score, pink esthetic scale (PES) score and incidence of complications were compared between the two groups.Results:The total effective rate of study group was 97.56% (40/41), which was higher than that of control group (90.49%, 33/41), P<0.05. There was no significant difference in the amount of marginal bone resorption and elevation of maxillary sinus floor between the two groups ( P>0.05). The intact rate of implant in study group was 92.68% (38/41), which was higher than that in control group (90.73%, 29/41), P<0.05. Three days after surgery, VAS score in study group was lower than that in control group, PES score was higher than that in control group ( P<0.05). The complication rate of the study group was 7.32% (3/41), which was lower than the 24.39%(10/41) of the control group ( P<0.05). Conclusions:Flapless osteotome maxillary sinus elevation minimally invasive implant combined with Bicon short implant system in the treatment of maxillary posterior single tooth loss can improve the therapeutic effect, improve clinical symptoms, improve the integrity of the implant, relieve postoperative pain, and enhance aesthetic, with safety.
