摘要目的:分析经腹腔镜全直肠系膜切除术(TME)联合改良Bacon术治疗低位直肠癌的效果。方法:抽取2016年2月至2021年1月河南科技大学第一附属医院收治的低位直肠癌患者50例，按照随机数字表法分为对照组和研究组，每组25例。对照组采用经腹腔镜TME治疗，研究组采用经腹腔镜TME联合改良Bacon术治疗。比较两组围术期情况、肛门功能(Wexner评分量表)、术后恢复情况及并发症发生率。结果:研究组术中出血量[(65.03±5.89)ml]少于对照组[(72.48±7.06)ml]， P<0.05。术后12个月，两组Wexner评分均低于术后6个月( P均<0.05)，但组间比较差异未见统计学意义( P>0.05)。研究组首次下床活动时间、住院时间均短于对照组( P均<0.05)。研究组吻合口瘘发生率(0.00%，0/25)低于对照组(24.00%，6/25)， P<0.05。 结论:经腹腔镜TME联合改良Bacon术治疗低位直肠癌可减少术中出血量，改善患者肛门功能，有利于降低术后吻合口瘘发生率，促进患者术后恢复。

abstractsObjective:To analyze the effect of laparoscopic total mesorectal excision (TME) combined with modified Bacon operation on low rectal cancer.Methods:Fifty patients with low rectal cancer treated in the First Affiliated Hospital of Henan University of Science and Technology from February 2016 to January 2021 were selected, and they were divided into the control group and the study group according to random number table method, with 25 cases in each group. The control group was treated by laparoscopic TME, while the study group was treated by laparoscopic TME combined with modified Bacon operation. The perioperative conditions, anal function assessed by Wexner scale, postoperative recovery and complication rates were compared between the two groups.Results:The intraoperative blood loss in the study group was (65.03±5.89) ml, less than the (72.48±7.06) ml in the control group ( P<0.05). Twelve months after operation, the Wexner score of the two groups were lower than those 6 months after operation (all P<0.05), but there was no significant difference between the two groups ( P>0.05). The time of first ambulation and hospitalstay in the study group were shorter than those in the control group (all P<0.05). The incidence of anastomotic leakage in the study group (0.00%, 0/25) was lower than that in the control group (24.00%, 6/25), P<0.05. Conclusions:Laparoscopic TME combined with modified Bacon operation in the treatment of low rectal cancer can reduce intraoperative blood loss, improve anal function, reduce the incidence of postoperative anastomotic leakage, and promote postoperative recovery.
