摘要目的 探讨血清肌钙蛋白Ⅰ联合心脏Tei指数对小儿重症肺炎心肌损害及心功能的临床评估效果.方法 选取本院自2015年3月至2017年7月收治且需住院治疗的80例肺炎患儿作为研究对象,根据《小儿内科学》重症肺炎诊断标准将患儿分为重症肺炎组和一般感染组,每组40例,比较两组患者的血清肌钙蛋白Ⅰ水平和心脏Tei指数.结果 经检测,重症肺炎组患儿的血cTnI为(1.88±0.53)μg/L、心脏Tei指数为(0.46±0.17);一般感染组患儿的血cTnI为(1.13±0.43)μg/L、心脏Tei指数为(0.28±0.09~所以重症肺炎组患儿的血cTnI和心脏Tei指数均显著高于一般感染组(P＜0.05).重症肺炎组合并心力衰竭的患儿的血cTnI为(2.98±1.33)μg/L、心脏Tei指数为(1.43±0.59);不伴有心力衰竭患儿组的血cTnI为(0.93±0.78) μg/L、心脏Tei指数为(0.29±0.11),重症肺炎组合并心力衰竭的患儿的血cTnI水平显著高于不伴有心力衰竭患儿(P＜0.05),但两组患儿的心脏Tei指数比较差异无统计学意义(P＞0.05).结论 血cTnI水平和心脏的Tei指数对小儿重症肺炎心肌损害以及心功能评估的意义重大,且两者呈正比,灵敏度高,特异性强,方便且安全性好,可在临床上广泛应用.

abstractsObjective To evaluate the clinical effect of serum troponin Ⅰ combined with cardiac Tei index in the assessment of myocardial damage and cardiac function in children with severe pneumonia.Methods 80 cases of children with pneumonia admitted to our hospital and requiring hospitalization from March 2015 to July 2017 were selected for the study,and were divided into severe pneumonia group and ordinary infection group according to the diagnostic criteria of severe pneumonia in Pediatric Internal Medicine,40 cases in each group.The level of serum troponin Ⅰ and cardiac Tei index of the two groups were compared.Results The level of serum troponin Ⅰ and cardiac Tei index of the severe pneumonia group were significantly higher than those of the ordinary infection group [(1.88±0.53)μg/L vs.(1.13±0.43)μg/L,(0.46±0.17) vs.(0.28±0.09)] (P＜0.05).The level of serum troponin Ⅰ of the severe pneumonia group with heart failure was significantly higher than that of the severe pneumonia group without heart failure [(2.98±1.33)μg/L vs.(0.93±0.78)μg/L] (P＜0.05),but there was no statistically significant difference in cardiac Tei index between the two groups [(1.43±0.59) vs.(0.29±0.11)](P＞0.05).Conclusion The serum cTnI level and cardiac Tei index are of great significance in the assessment of myocardial damage and cardiac function in children with severe pneumonia,and they are of high sensitivity,specificity,convenience,and safety,which can be widely used in clinical.
