低速直涡轮机配合高速涡轮机拔牙术治疗下颌复杂阻生智齿患者的临床研究
Clinical study on low speed straight turbine combined with high speed turbine extraction in the treatment of complex impacted mandibular wisdom teeth
摘要目的 探讨低速直涡轮机配合高速涡轮机拔牙术治疗下颌复杂阻生智齿患者的效果及安全性.方法 选取本院2016年11月至2018年6月下颌复杂阻生智齿患者86例,随机数字表法分为两组,各43例.对照组采取传统锤凿法拔牙术,研究组采取低速直涡轮机配合高速涡轮机拔牙术.统计两组治疗情况[手术时长、肿胀程度、张口受限程度、牙槽窝完整性、疼痛程度(VAS)]、治疗满意度、并发症(感染、干槽症、断根)发生率.结果 研究组手术时长、肿胀程度、张口受限程度、牙槽窝完整性、VAS评分分别为(13.18±2.19) min、(14.86±0.95)mm、(3.46±1.01) mm、(1.81±0.22)分、(1.83±0.51)分,均低于对照组,差异均有统计学意义(均P< 0.05);研究组治疗满意度93.02%(40/43),高于对照组74.42%(32/43),差异有统计学意义(P<0.05);研究组并发症发生率9.30%(4/43),低于对照组25.58%(11/43),差异有统计学意义(P<0.05).结论 联合采取低速直涡轮机配合高速涡轮机拔牙术对下颌复杂阻生智齿患者予以治疗,可减少手术创伤,保证牙槽窝完整性,术后疼痛感较轻,且治疗满意度较高,利于降低并发症发生率,安全性较高.

abstractsObjective To evaluate the efficacy and the safety of low-speed straight turbine combined with high-speed turbine extraction in the treatment of complex impacted mandibular wisdom teeth.Methods 86 patients with complex impacted mandibular wisdom teeth from November 2016 to June 2018 were randomly divided into control group (n=43) and study group (n=43).The control group was treated with traditional hammerchisel extraction,while the study group was treated with low-speed straight turbine combined with high-speed turbine extraction.The duration of operation,degree of swelling,degree of limitation of mouth opening,integrity of alveolar socket,degree of pain (VAS),treatment satisfaction,incidence of complications (infection,dry socket,root cutting) were compared between the two groups.Results The duration of operation,degree of swelling,degree of limitation of mouth opening,integrity of alveolar socket,VAS in the study group were (13.18±2.19)min,(14.86±0.95) mm,(3.46±1.01) mm,(1.81±0.22) points,(1.83±0.51) points,which were less than those of the control group (P<0.05).The satisfaction rate of the study group was 93.02% (40/43),which was higher than that of the control group [74.42% (32/43)] (P<0.05).The incidence of complications in the study group was 9.30% (4/43),which was lower than that in the control group [25.58% (11/43)] (P<0.05).Conclusion Low speed straight turbine combined with high speed turbine extraction in the treatment of complex impacted mandibular wisdom teeth can reduce surgical trauma,ensure the integrity of alveolar socket,reduce postoperative pain and the incidence of complications,and has higher degree of treatment satisfaction and higher safety.
