摘要目的 探讨富含半胱氨酸61(Cyr61)蛋白、p53蛋白在胆囊癌组织中的表达及其临床意义.方法 选取太钢总医院病理科收集70例胆囊癌组织(胆囊癌组)、选取慢性胆囊炎组织70例(对照组),分别采用免疫组织化学染色检测胆囊癌组和对照组中的Cyr61蛋白、p53蛋白表达,并分析不同组织分化、不同Nevin分期、是否发生淋巴结转移、是否侵犯周围组织胆囊癌组织中的Cyr61蛋白、p53蛋白阳性表达率差异.结果 胆囊癌组织中的Cyr61蛋白、p53蛋白阳性表达率均高于对照组,差异有统计学意义(64.29％比24.29％、61.43％比12.86％,P＜0.05);胆囊癌组织中的Cyr61蛋白与p53蛋白阳性表达无明显相关(r=-0.223,P＞0.05);低分化、Nevin分期(Ⅲ期)、发生淋巴结转移、发生周围组织侵犯的胆囊癌组织中的Cyr61蛋白阳性表达率分别为77.42％、86.36％、74.47％、75.56％高于高中分化(53.85％)、Nevin分期(Ⅰ期+Ⅱ期,54.17％)、发生淋巴结转移(43.38％)、发生周围组织侵犯(44.00％,P＜0.05).低分化、Nevin分期(Ⅲ期)、发生淋巴结转移、发生周围组织侵犯的胆囊癌组织中的p53蛋白阳性表达率分别为77.42％、86.36％、78.26％、72.00％高于高中分化(48.72％)、Nevin分期(Ⅰ期+Ⅱ期,50.00％)、发生淋巴结转移(53.19％)、发生周围组织侵犯(55.56％,P＜0.05).结论 胆囊癌组织中Cyr61蛋白、p53蛋白表达水平上调,相关因子水平的变化与癌组织分化、Nevin分期、是否发生淋巴结转移及是否侵犯周围组相关明显.

abstractsObjective To investigate the expression of cysteine-rich 61 (Cyr61) protein and p53 protein in gallbladder carcinoma and its relationship with tumor development.Methods We selected 70 cases of gallbladder carcinoma (gallbladder cancer group) collected from Department of Pathology,January 2015 to October 2017 in our hospital,and selected 70 cases of chronic cholecystitis (control group).Immunohistochemical staining was used to detect gallbladder carcinoma and The expression of Cyr61 protein and p53 protein in the control group was analyzed,and the differences in the positive expression rates of Cyr61 protein and p53 protein in different tissue differentiation,different Nevin stages,lymph node metastasis,and invasion of gallbladder tissue in the surrounding tissues were analyzed.Results The positive rates of Cyr61 protein and p53 protein in gallbladder carcinoma were 64.29％ and 61.43％,respectively,which were significantly higher than those in the control group (24.29％ and 12.86％,respectively).The difference was statistically significant (P ＜ 0.05);in gallbladder carcinoma tissues There was no significant correlation between the positive expression of Cyr61 protein and p53 protein (Speraman's rank correlation coefficient =-0.223,P ＞ 0.05);The positive expression rates of Cyr61 protein in gallbladder carcinoma tissues with poor differentiation,Nevin stage (stage Ⅲ),lymph node metastasis and peripheral tissue invasion were 77.42％,86.36％,74.47％,and 75.56％,respectively.It was higher than 53.85％ of high school differentiation,54.17％ of Nevin stage (stage Ⅰ + Ⅱ),43.38％ of lymph node metastasis,and 44.00％ of surrounding tissue invasion (P ＜0.05).The positive expression rates of p53 protein in gallbladder carcinoma tissues with poor differentiation,Nevin stage (stage Ⅲ),lymph node metastasis and peripheral tissue invasion were 77.42％,86.36％,78.26％,and 72.00％,respectively.It was higher than 48.72％ of high school differentiation,50.00％ of Nevin stage (stage Ⅰ + Ⅱ stage),53.19％ of lymph node metastasis,and 55.56％ of surrounding tissue invasion (P ＜ 0.05).Conclusion The expression levels of Cyr61 protein and p53 protein were up-regulated in gallbladder carcinoma tissues,and the changes of related factors were correlated with cancer tissue differentiation,Nevin stage,lymph node metastasis and invasion of surrounding tissues.
